Controlled Substance Limitations

All Scheduled Controlled Substances must come from and be managed by a Prescribing Provider with Clendenin MD.

Mental Health Conditions Not Treated

We don’t currently treat Schizophrenia, certain personality disorders, severe mental illnesses that require inpatient care, or patients looking for therapy only.

Ketamine and Substance Abuse History

ADHD Evaluation Requirement

All of our ADHD patients will undergo QbCheck by remote testing to aid in their diagnosis and progress.

Late & No-Show Policy

More than 10 minutes late for scheduled appointment will be considered a no show and there will be a $25 rescheduling charge.
