Important Notice:
Clendenin MD requires all ADHD and Anxiety and/or Depression patients to provide a drug test result that includes "AMP" (Amphetamine) and "KET" (Ketamine) with their name and the date clearly labeled. This drug test result must be submitted prior to each visit, every three months.
Additionally, patients are required to complete the ASRS Anxiety and/or Depression Scale Symptoms Checklist form, which will be provided every three months.
